Non-captured Waking Events
I’ve had the Airsense 10 for 2 weeks. Dramatic improvement in dry mouth over my AirMini which is now just for travel. Also, with the AS10 comes the ability to use OSCAR software which allows breath by breath reporting. Game changer! I have very good reduction in captured apneas, down to about 6 per night. Almost exclusively CA. None of them wake me up. However, I regularly have non-captured events that do wake me up. These events, I call waking events (WE), happen about once every other night. They look very similar to a CA with key differences. Captured CA events last about 15 seconds, the non-captured WE last about 6 seconds (hence non-reported). BIG difference is in reduced respiratory rate duration. Only about about 12 seconds in CA, but up to 20-30 minutes in WE! Respiratory rate in CA goes from about 16 down to 7 and back up in less than 20 seconds. Similar drop in WE about 15 down to 8, but many minutes before coming back up. I find I can not get back to sleep until RR returns to normal range (15 ish). Increases in minimum pressure up to 10 did not resolve. Pressure is flat during events. Any thoughts would be appreciated. I am so close to a full nights sleep.

Welcome to the board - can you show us a couple of full nights of OSCAR daily view w/ calendar minimized and zoom in on one of the events you describe please?

Thanks so much for your reply. First image will be last nights full screen. Second will be a representative captured CA which shows quick RR recovery. Final is the waking event. Note shorter duration, and RR staying down. I took off the cpap at the end of the full screen, but continued to track resp rate on autosleep app, and it stayed low for about 30 minutes.

Given that this unmarked event was at the end of your sleep cycle it was likely in REM and if I had to guess it looks like a spontaneous breath hold as you change positions.
One thing I noticed about your charts is I think your biggest opportunity for improvement is to solve your mouth leak as your mouth is open for almost the entire night. Are you using mouth tape or a chin strap? If not, I'd go to your local CVS, Walgreens or Target and pick up some cloth medical tape for $4.50 or so and tape your mouth tonight. Considering the amplitude and duration of your mouth leaks this is likely fragmenting your sleep and may be giving your body a hard time in normalizing oxygen / carbon dioxide overnight.

Thanks for your input! These events do occur in the middle of the night as well, but do tend to happen during REM. I experience them with or without CPAP. They were one of the original issues that sent me to the sleep clinic for the test. I do use both a chin strap and "Hostage" mouth tape. I notice now that those leak rate numbers have gone up some since experimenting with increase to pressure minimum setting. To be honest, I have no idea what is a good or bad number on leak rate, and will look into that. It seems that the low respiratory rate over 30 minutes might be a clue into some other issue as well. Again, I do appreciate your feedback.

Again, I do appreciate your feedback. Good luck hunting leaks, ideally you'd want to get your 95% leak rate to 0.00 but without a full face mask this can be hard to do.

You were right Gainerfull, I woke up last night with a steady stream of air escaping around the far corner of my "hostage tape". I always get a smily face from ResMed on leakage, so I did not know it was an issue. Below are my stats for last night, the last week, and the last 2 weeks respectively. While these do not look horrific, I am going to work on addressing. At higher minimum pressures I can feel air in my mouth. I have success pressing my tongue to the roof of my mouth preventing that, but it doesn't stay all night. Eventually it must put enough pressure on the hostage tape to cause a leak. I will try the tape you recommended tonight, and might consider lowering my minimum autoset pressure. Thanks again for your experience and advice.

Glad to hear you discovered the source of the problem. I wouldn't adjust your pressure settings unless you absolutely have to, though. One thing that helped me is turning climate control to "manual" and setting the humidity level to "1'. Also, when you apply your tape, purse your lips like you're kissing and then apply the tape so it gets a good seal and after this revelation you might even cut a strip of hostage tape in half and do another layer on the corners of your mouth. If you can inflate your mouth without air escaping then you'll know you have a good seal. Another option is to trial a true full face mask like Resmed F10 or F20 but make sure to get a fit pack so you get all of the different sizes of mask pillow. My avg leak rate for this month is 0.10%. I don't recommend any of the hybrid masks as they are difficult to fit, use and control leaks.
