To get your request 1 to work, as in prevent CA, the ResMed ASV will get you closer to the required target than Philips Respironics machines. The PR is a reactionary device, meaning it's doing to adjust after the event starts. ResMed will sense it's about to happen and step up therapy to eliminate CA to almost zero occurrences.

Hey there SarcasticDave ... thanks very much for your recommendation. Interestingly, I think it's one of the ResMed machines that some people have suggested may do a better job with UARS versus some of the other machines.
We are passing on the replacement for my son's recalled Philips machine because there appears to be a good chance it may not be supported by Oscar, so the ResMed is looking like a good option. Appreciate it.

- Much worse since starting on CPAP/ASV. 100% Compliant. ADDITIONAL SYMPTOMS/ISSUES: - Rhinitus: Possibly allergic, getting tests. Could be non-allergenic. - Nostrils appear to "collapse" when inhaling quickly (more than other people) - Expiratory palatal prolapse - REGULARLY recurring dreams of pulling gum or taffy from mouth, throat, teeth; drowning; falling; chasing people; sand in mouth; nose and face covered by something; feel like head is breaking through a "membrane". Upon waking: No refreshment; sweating; skin burning; tongue thrust between teeth - Weird breathing when simply lying down, or falling asleep - like holding breath. - SEVERE fatigue. Struggle to even get out of bed, can't sustain physical or mental activities for long

Hi. Just bumping a recent post. I got a recommendation for dealing with our first objective of eliminating centrals (get a new ResMed machine), which I will consider.
In the meantime, it seems like we probably should move on to our second objective, which is to reduce my son's AHI which is currently above 20 most nights. Any suggestions?

Bumping this again. Note we are skipping Step 1 (eliminate central apneas) for now and concentrating on reducing AHI.

RE: UARS Treatment [Using Philips ASV]
Hi. My son recently reduced his AHI from about 20 or so, on average, by reverting to some previous settings. However, it is still in the 7-10 range, and more importantly, does not feel any better at all. His fatigue is totally debilitating. He also has some other medical issues which are likely playing a role here.
I've added a bunch of these symptoms to my profile, which is included at the end of this post. I am posting a screenshot to reflect the new settings and his response to them. I am also posting a couple of detailed Flow Rate graphs. One shows a repeating pattern of breaths that I would like to be able to interpret. It looks to me like he has a long pause at the end of his exhalation, before he inhales. I zoomed this because it seems to be very representative of his "normal" breathing throughout the night. But it does not seem to be a healthy waveform to me. The other Flow Rate graph zooms in on a typical disruptive event in his sleep. The vertical ticks are Timed Breaths. Again I am wondering whether this info can help anyone offer some insight or advice. Thanks in advance for any help that anyone might be able to offer.

This looks to me to have the pattern for Positional Apnea. Has that and chin tucking been discussed yet? This is where the person's chin tucks towards the chest and kinks off the airway externally.

Hey Dave. Thanks (again).
I think you suggested the same recently, but we haven't picked up a collar yet. I will probably pick one up tomorrow or Monday and try it. I guess that's the only way we'll know. I do have a security camera with infrared that I just set up to record my son sleeping. I haven't had a chance yet but I am going to see if I can relate anything in that to his Oscar data. But probably just as well to try the collar first. Thanks very much.
