hammer44 - ASV Therapy Equipment Question
New Member,
transitioned from Dreamstation 3000 inJuly. Now have Resmed Aircurve10 ASV. Doesnt seem like it is working the same as the other. I have complex sleep apnea with 26 episodes per night during the sleep study originally. Ahi now 10 (found from calling physician office. Also have AFib. My sleep physician is retiring and now I am trying to get them to fix the machine which doesnt show any sleep reports. and connectivity bars are greyed out. Anyway sorry I didnt read this before about “not accepting a new machine til you read this.”
I dont think this is the right machine for me. Dreamstation was an APAP and I think this one is not? any advice welcome. Thank you.

RE: hammer44 - Therapy Equipment Question
What is a Dreamstation 3000? The only "3000" I could find is Philips Respironics Cough Assist CA3000.
Post a screenshot of your daily charts using OSCAR so we can see what is going on and help you,.

You should be able to enter the clinical settings menu and scroll through. The only important settings are mode (ASV or ASV-auto), EPAP or EPAP min, EPAP max in auto mode, PS min and PS max. To see clinical settings, press the control knob and home button at the same time, select settings, and then record what you see. If you continue down the menu, you will see Essentials with a setting of on or plus. You want to select Plus to see more information in the on-screen sleep report. Be sure ramp is off, and humidity settings make sense.
RE: hammer44 - Therapy Equipment Question
Ok, the mode is Asv auto, epapaitomode max is 8, epap is min 8. max 8,,
PS min is4, MMax 16. average Ahi in last 7 days is 6.7.
had to get this thru a call to provider. for noe. thank you
RE: hammer44 - Therapy Equipment Question
With the ASV, all events are registered as UA (unidentified apnea), and they are nearly always obstructive, and you have enough PS range to obliterate any central apnea. Obstructive apnea with ASV can either arise from EPAP being too low or a positional issue where your head and neck are poorly aligned and that obstructs the airway. The default settings for the Resmed Aircurve 10 ASV are EPAP min 5.0, EPAP max 15.0, PS min 4.0, PS max 15.0. Your machine may be in ASVauto mode, but your settings make EPAP fixed, which is simple ASV mode. The titration protocol and decision tree are below. Logically, without data, I have to recommend that you modify settings to EPAP min 8.0, EPAP max 14.0, PS min 4.0, PS max 15.0. All we are doing is enabling ASV auto to do its job by allowing a higher EPAP if needed. It would be helpful to know what settings were on the Philips BiPAP Auto SV. This is a link to the positional apnea wiki. With data we would quickly see if your apnea events are in close clusters which is usually positional, but if they don't show that pattern, then we necessarily move to pressure. https://www.apneaboard.com/wiki/index.ph...onal_Apnea
My expectation is that if you change the EPAP max to the machine can respond to obstructive events, you will see fewer events. If not, then a soft cervical collar may be in your future. Either way, finding a means to access the SD card and uploading to Oscar would be helpful.

RE: hammer44 - ASV Therapy Equipment Question
Yep, close enough to default like this will likely improve things pretty quick. The ASV may seem to be a complex machine, even so you'll feel a difference rather soon, either good or worse. Tuning won't take long either.
PS when you check in with results next, give us some details on how ASV therapy felt, highlight differences, what was better or worse, etc. It'll help us steer the settings for you.
