Can you guys help me optimize my settings?
Hey guys! I´ve been a apap user since may/24. I was diagnosed with 45,8 AHI events.
I didnt have a tiltiration study. The technician that sold me the resmed airsense 10 autoset provided a min 4 and max 8 pressure and I´ve been tinkering with the pressures ever since.

I feel like increasing my pressure resulted in more CAs, perhaps it would be best to lower the pressure again and try a more narrow band of pressures, or maybe use the machine in cpap mode. Either way, I would appreciate your feedback if possible.

Below are the cpap statistics, a good and most recent (no bad) night of sleep.

If anything is missing, please let me know.

You are consistently using EPR full-time at setting 3, and have used minimum pressures ranging from 4.6 to 6.0 cm, and maximum pressures from 7.0 to 11.0. Your Airsense 11 Autoset is capable of producing a minimum pressure of 4.0, so with EPR at 3, we usually want to see a minimum pressure of 7.0/4.0 (inhale/exhale). Your most recent results on 1/1/25 had an average pressure of 8.6/5.6 (inhale/exhale) and resulted in the worst AHI you have yet experienced. It appears you do better with lower pressure, and I think we need to reduce EPR accordingly to avoid some of the CA events that we see with higher pressure and EPR. Your results from January 2 shows a 0.00 AHI with a median pressure of 6.0/4.0, so that is the recommendation.

Minimum pressure 6.0, Maximum pressure 9.0, EPR full-time at setting 2. Any chance that a New Year celebration affected your results on 1/1?

My absolute worst AHI since I started my treatment has been 5,02 in november 11th, 2024:
   

I only managed to get a 0.0 AHI twice: july 14th (4.6 - 7.6):
   
and september 2nd (4.6 - 8.6) as my previous post.

Regarding new years, I slept pretty late and sipped a bit of champagne, might have something to do with that... But its not too far from december averages:
   

I thought you would recommend a fixed pressure, but perhaps you didnt see anything that might suggest I needed that.

Either way, I ll change to 6-9 with epr at 2. Thanks a lot for your feedback!

Your latest charts suggest you have the potential for pretty high flow limits that look positional. Let's see what the EPR 2 does, but we could end up trying some fixed pressures.

Everything looks good.  Centals are just you holding your breath.  We all do it when we roll over in bed or pull up sheets.  Usually it does not show up as as central because it did not last long enough (at least 10 seconds) plus central apnea are consistently inconsistent meaning they come up on night and not the next.   Don't worry about a few, if they get more numerous ask again with new OSCAR.

I would make one small change in you settings.

min 7
Apnea (80-100%) 10 seconds, Hypopnea (50-80%) 10 seconds, Flow Limits (0-50%) not timed  Cervical Collar - Dealing w DME - Chart Organizing

For you, this looks pretty good at 6-9 EPR 2. Flow limits came down to 0.15 and AHI is good. I think there is a positional issue here, but other than the periodic obstruction and flow limitation, it's pretty hard to pick out.

Seems like a good number. I ll keep these as baseline and try to change a few things like sleeping on my side or using a soft cervical collar to see the difference.

I´m chasing that elusive 0.0 AHI haha

Good choice raising he minimum +1. Good luck with the 0.0. I get it on rare occasions, but don't really set that as a goal.
