Travel insurance doesn't care. The biggest cost to the insurance company is medical or evacuation or transport (of dead body). The chances of these things happening while on vacation are very, very small because very few people buy travel insurance unless they are vacationing and they don't vacation if they are really sick. Usually. My neighbor had to sign off of some coverages because he was in the middle of cancer treatment for bladder cancer (his doc said it was ok to travel). He traveled and returned without incident, so that was good.
