RE: Lowenstein Prisma 20a - How can I get acclimated?
The algorithm ResMed uses is different from Lowenstein.
ResMed does not start the transition from EPAP to IPAP until you start inhaling. The Lowenstein does the transition to IPAP sooner, starting during the latter part of your exhale. This is to help keep the airway from collapsing during exhale which is where most OAs occur. This is how most people can use a lower pressure and get the same or better therapy with the Lowenstein.
Try softPAP2 at first so you get the pressure support you need (due to the flattened inhalation curve you mentioned) and then go to softPAP3 if needed and/or you are comfortable with softPAP2.
Give it some time, you are likely just used to the ResMed therapy and will adapt to the Prisma in time.

RE: Lowenstein Prisma 20a - How can I get acclimated?
In time means many bad nights of therapy! I tried for an hour or two both nights so far and was unable to fall asleep even though I was very tired. I do understand the algorithm and its benefits, but I wish there was a way to turn it off so it acts like the Resmed, then apply the early IPAP during when I am asleep. ANyone have any ideas how to do this? I was starting out at 8 with 3 EPR with my Resmed with AUto Ramp.

You explained what I was trying to describe as a "smoother transition from exhale to inhale" in my earlier post. You explained it much better. Likely why my AHI and restfulness have improved over the over the past few nights and all with a lower pressure than my ResMed AS10.

Isn’t a lower pressure bad since most people can’t do less than 6cm?
