I thought you might have tried Sleeprider's setting recommendation on Friday or Saturday night. Is there a reason you didn't want to try it? The result would have provided him with the information to help you.

Crimson Nape - I am going to begin it tonight!  I had a very busy and important week (including the weekend) so didn't want to potentially have a horrific night sleep (although 5/1 was pretty bad but I played the odds of something I know versus the unknown!)

Following Sleeprider's suggest to trial some change in settings:
Autoset standard mode, 
minimum pressure 9.0,
maximum pressure 12.0
EPR on full-time 
EPR setting 3

Attached are two nights in a row of these settings.
5/5 AHI 4.39 felt 5/10.  I totally lost my train of thought during a conference call which is totally uncommon for me.  I also called my kids by the wrong name a few times.   Too-funny
 
5/6 AHI 9.02 felt 5/10 overall feel pretty tired today.  I haven't had any important meetings so can't comment on any brain fog.

Okay, EPR 3 definitely kicked EPR to a much lower rate, but CA increased and resulted in dementia. Not a good outcome. We need a different solution. If it were up to me I'd put you on a ASV with enough pressure support to resolve the flow limits and the capability to resolve CA events. It's not in my powers to do that, so I think you back off to previous settings that did not result in these adverse consequences. EERS would probably resolve the CA events, but that is kind of a DIY task. https://www.apneaboard.com/wiki/index.ph...ace_(EERS)

All,

Back to my previous settings (CPAP, Fixed 8.0, EPR 1) and feel better. 
5/7 attached.  AHI 5.34.  Feel 8/10
5/8 attached.  AHI 3.37.  Feel 7/10

Sleeprider:  I will review over EERS and and ASV from the acronym list.

Are my OSCAR data showing a very specific and easily identified diagnosis such as central sleep apnea rather than obstructive?  Is this normal for a healthy normal BMI late 30's male? I'm happy to experiment (such as what we've been doing with this thread) and even buy expensive equipment or remedies if there is a chance it will provide a better outcome.  As mentioned previously I work for quality of life and although I am totally functional as it currently stands, I would certainly prefer much better sleep even if there is quite a steep upfront cost involved to figuring it all out.

Apneavolt, when I invited Crimson Nape to step in on this thread, it was because he has much more knowledge than I and I hate to spend other peoples money without a positive assurance it will be the best investment. I have continued to follow this thread to further my own knowledge.

The rock and the hard spot Crimson Nape first mentioned. Was trying to get the most effective use out of your current machine before recommending an expensive upgrade to a model that would have better chance of success.

SleepRider is very well recognized on this forum for his knowledge of equipment capabilities and the needs of individuals' specific PAP requirements. So his recommendation of ASV is the equipment you will need to pursue.  ResMed AirCurve 10 ASV.

Sleeprider can point you to the vendor and discounts that will provide the most effective use of your investment. I am not cognizant of that information.

I have no knowledge or experience with ASV, so there again I will defer to other forum members for that. I will continue to follow for my own edification.

I would first look at Supplier #2, and search the DotMed website. DotMed is an eBay for medical equipment. I used the search term, "Resmed ASV", and just found several. If nothing else, it will provide a knowledge base on pricing. I have purchased several CPAPs for the DotMed website, with the most notable vendor being Prontodepot. I've never had any problems. In fact, I've received machines that were much better than expected.

Thank you all for the help. I have used the DotMed website to acquire a ResMed AirCurve 10 ASV. The seller is in my same state so I suspect I'll receive it in only a few days time.

When it comes in what settings do you suggest?

Do I simply use my SD card from my ResMed Airsense 10 or should I get a brand new SD card?

Congratulations!  As far as the SD card, if you are planning to use the AS10, then I would go on and get another SD card.  If not, format the current card prior to use in your AC10 ASV.  Also, before it first use, make sure to preform a complete factory reset.  This will erase any previous data that may be in the machine.  The last thing you want is someone else's data.

As far as settings; Sleeprider can provide a better recommendation than I can.  Anyway, see his POST HERE to get an idea of the ASV Titration Protocols.
