How long have you had the current machine? If you were to get a new machine, would insurance be involved in the purchase?

I would like to have you consider replacing the current machine with the ResMed AirCurve 10 VAuto, but not the ASV which specializes in Central Apnea.

Why the VAuto Dave? Your Central Apnea is very low on the tests, higher with therapy at home. So the doctor and insurance may not pay up extra for ASV. However, the VAuto I mentioned has an ace up its sleeve to help you. The VAuto has timing controls, specifically Trigger which will help the CA and treat the other events great.

Alternate plan is buy the ASV yourself and treat the Centrals you have. The ASV therapy will be requiring you to accept a different feeling to pressure delivery. It'll blow hard at times in Central Apnea combat.

What is your Trigger setting? Set it to High and let's observe the results.

Many times, setting the Trigger to High or Very High alleviates CA's.

Hello setting was at high , I actually had it at Medium , saw some threads about this  , and changed it to High . Two days later 24 Central Apneas per Hour.

Also P.S is around 3.6 , Which seems odd that is creating so many Centrals.

Hello Dave , I am sorry but isn't that the Device that I currently own. Air Curve 10 Vauto.

Trigger setting is Hight at the moment.

Do you still think BiPAP is needed when I do not feel lots of difference between CPAP and BiPAP therapy in terms of fatigue.

Also confused about how no pressure under CPAP took the Hypopneas away until they put me in 20/16 Bipap. 

Also why where 98% of Events Hypopneas. I thought I had sleep apnea not Hypopneas from a test back in 2018. Recent is 2023.

I doubt the Trigger setting is causing more CA's, more than likely your PS of 3.4 is the culprit.

You might want to go back to the settings you used on April 3rd.  It looks like you do much better with the lower pressure support. (PS 3, Epap 10).

You might even want to try lowering the Epap to 8 or 9 with PS 3.

IF your trigger is set to high set it to very high and see if that helps with the centrals.  I had quite a few centrals until I got the VAUTO and changed the trigger setting.  I have seen it help many people here that had a lot of CA events.  Alll it does is triggers the pressure quicker.
Apnea (80-100%) 10 seconds, Hypopnea (50-80%) 10 seconds, Flow Limits (0-50%) not timed  Cervical Collar - Dealing w DME - Chart Organizing

Thanks , will do. Makes me question if I even need a BiPAP when PS of 3 seems to work better for me in terms of centrals. Wouldn't a CPAP give me the same PS with EPR= 3?

Also any one have any thoughts on my Sleep Study , and the vast majority of events being Hypopneas?

The VAuto is still the better machine, it has timing controls that an AutoSet doesn't. If you're editing trigger on the VAuto to combat Central Apnea, there's your reason 1 to keep VAuto.
