Air Pressure Issue

Air Pressure Issue
I use ASVAuto and I read on lankylefty’s site that it ALWAYS should be set at 6 and 20. My issue is that when I use a full face mask it starts fine but like an hour in I get so much air push in which I can't take in so it leaks from the sides no matter how loose or how tight I have it. Is that true? Is there a way from stopping this from happening? When I use the Bleep it's fine there's 0 leaks, problem is am a mouth breather and the taping on and on it's a hassle, I'm disabled so someone else has to do it for me and you all know how burdensome that is. Is there a way around this issue? I need some help here please. Thank you all ?

Don't have Oscar , no computer with me. And the ones on sleephq are from the Bleep mask which works just fine. It's when using FFM or Hybrid. Here's more detail.

I was using the Evora then the Air touch F20 door a good year or so. Now I can't, it's like I'm back on a V60 machine or bipap that the machine doesn't let me finish my exhaling and it starts pushing Air back to me and I can't inhale all the air and it goes out through the sides. I've try using every mask out there you've seen my list, in fact tried again the F40 last night same thing. I even lowered settings to min max ps 5 and 10 and Max min epap to 6 13 if I can remember correctly on the road ATM. Tried back the F20 nothing. Got both quattro the original and FX same, got the Lena and same. Only thing that works fine is the Bleep Eclipse but the tapping is a hassle. I don't know what to do. Don't want to try the Airsense because I have centrals. I'm desperate and don't know what else to do. I m not done exhaling when air is rushing in, also when I get to exhale fine and it isn't pushing Air as soon as I start inhaling boom it's just forced air like if I'm not breathing, I'm freaking awake I'm breathing fine. So I have to force myself to adjust to what the darn machine is doing instead of the other way around. I'm seriously desperate.

RE: Air Pressure Issue
First of all, Lefty Lanky has been spewing some really bad advise lately, and you should never use his opinion in lieu of your titration result of EPAP min 9.0, EPAP max 15.0, PS min 3.0 and PS max 15.0 without a qualified evaluation or other data that can identify reasons to make a change. What are your current settings? Using a Vauto between 6.0 and 20.0 is an incomplete settings suggestion, but makes sense, however it is an absurd description of ASVauto settings. Your pressure needs and mask settings will change from a nasal pillows or Bleep, to a full face. A full face mask is very difficult to use with ASV due to the larger mask volume and the flexing caused by the pressure changes inherent in ASV. Let's start with an accurate understanding of your settings and a better description of your perceived issues.

RE: Air Pressure Issue
Hi Vako, sorry to hear of your discomfort lately.

I have a mask suggestion, only because i never see it listed on here, and i use it with no issues really, and you appear to be pretty stressed....

Phillips dreamware nasal mask,   no pillows.

Very comfortable, low leaks for sure.

It is never mentioned, not sure why, thought i would throw that out there.

I have a large hump on bridge of my nose used to use the resmed nasal caused marks, leaks etc.
Then dreamware full face, and it was ok too, but i went the mouth taping route, being a mouth breather all my life ( but starting to fix that i hope)

So i discovered the dreamware nasal and have not looked back.
Oh, and i just recently switched my setting to full face, still using nasal mask, and definitely gentler pressure.

Doesn't work for me. I actually have 5 of them in their package new. I do have to use Nasal the problem is I'm a mouth breather so taping is a hassle.

RE: Air Pressure Issue
You are using a ResMed ASV Auto? Pressure 6-20 is an incomplete setup. You need to have mode first. And if you're using ASV mode, you'll have static/single EPAP, and 2 PS numbers, and will give by mathematics 2 IPAP numbers. If it's ASV Auto mode, there's 2 EPAP, 2 PS, and 2 IPAP.

So you'll need to fill in the blanks for us.

Mode ____
EPAP ____
PS _______
And we can figure IPAP from this, as EPAP plus PS equals IPAP.

RE: Air Pressure Issue
With EPAP 5-12 and PS 3-13, you'll have IPAP 8-25. And I'll make a guess, if you say it starts at 4, you're using Ramp.

The Ramp itself is probably part of your issue. This Ramp time will reduce your overall therapy time, and will set you up where there's an abrupt transition between Ramp and therapy pressure.

If you must use Ramp, then it'll need to be equal to your min EPAP, but this also is less than ideal. Better is finding out how to prepare for therapy without the Ramp. Don't put the mask on if you're breathing faster than resting rates. The ASV will pace your breath rate for a rolling 3 minute window.

As for pressure settings, it's going to be hard to assist without a chart.

Yes I use ramp, I see your point looks like when ramp ends is when all heck breaks loose thing is ASV is supposed to adapt to your breathing pattern not the other way around even when I'm wide awake I'm struggling to keep up with it because the breathing pattern it shoots at me it's not mine I'm either having to exhale earlier or inhale while I'm not even ready so after 15 minutes of struggling trying to keep up I just toss it away cuz the leaks drive me crazy. How can I show a chart of maybe 10mins? Is that going to yield anything? The only full charts I have are wish nasal which I have no problem with but the taping. Heading to hospital ATM to be inpatient for infection, imagine me asking nurses to tape my mouth because I'm disabled I can't myself.
